LUMBAR INTRADURAL DISC HERNIATIONS

Ankara Numune Hastanesi Nöroşirürji Kliniği, Ankara We present four cases of intradural lumbar disc herniations with their MRI characteristics. Two cases were operated on with a preoperative diagnosis of spinal intradural rumor and the others with diagnoses of lumbar disc herniations. Ventral aspect of lumbar dura was found to have dense adhesions with the posterior longitudinal ligament, and free fragments of nucleus pulposus were located between the rootlets of cauda equina with rupture of annulus fibrosus, posterior longirudinalligament and ventral aspect of lumbar dura. In retrospective evaluation MR images were not found to be characteristic of intradural disc rupture. However, intradural disc herniations should be considered in patients with clinical criteria, and intradural MRI lesions associated with intervertebral disc, and degenerative signal changes in the same disc level.
