Far lateral lumbar disc herniations differ from classic herniations, which occur on the medial side of the facet joint, by both of their clinical findings and surgical managements. They usually present with upper lumbar nerve root compression symptoms and diagnosis incidence have increased through computed tomography, magnetic resonance and recently magnetic resonance single-shot fast spin-echo sequences techniques. The posterolateral approach is preferred in surgical treatment of far lateral lumbar disc herniations since the facet joint was leaved intact. We report a case of the mentioned approach was used and discuss the related literature in this paper.
