Anterior Cervical Cage and Plate Application Techniques

The introduction of anterior cervical discectomy and fusion by Cloward and Smith-Robinson in the mid-1950s enabled surgeons
to use them as effective techniques in the treatment and expansion of various disease processes involving the cervical spine. Over
time, spine surgeons have come to realize the superiority of these anterior approaches. However, it is not always safe to provide
stability with interbody fusion. Grafts often collapse and become extruded. The result is nonunion and kyphotic angulation. The
incidence of such formations increases when the level of surgery increases, and also increases when an allograft is used. Despite
pioneering efforts to decompress and stabilize the cervical spine with the anterior approach, posterior decompression of the cervical
spine has remained the preferred approach. However, it should be noted that while the anterior approach generally allows fusion,
the posterior approach does not.
