²Polatlı Duatepe Hastanesi, Ankara, Türkiye The authors discuss their experience of treating basilar invagination by performing atlantoaxial joint distraction, reduction of the basilar invagination, and direct lateral mass atlantoaxial plate/screw fixation. They believe that joint distraction and firm lateral mass fixation surgery of basilar invagination is a reasonable and long-term reliable surgical treatment strategy for reducing the basilar invagination, restoring craniospinal alignment, and establishing fixation of the atlantoaxial joint. They also share their personal surgical experiences with pitfalls and technical difficulties of this strategy.
