Role of Radiotherapy in the Treatment of Atypical Meningioma

Atypical meningiomas have a higher risk of recurrence compared to benign meningiomas. The role of adjuvant radiotherapy after surgery for atypical meningiomas is controversial. Although radiation is administered mostly after subtotal resection (STR) or recurrence, there is no standard approach after gross total resection (GTR). Retrospective series with a limited number of cases have provided contradictory results. Adjuvant radiotherapy decreases local recurrence rates and improves overall survival after STR in most series, whilst also increasing local control rates after GTR. Following the publication of the recent results of two multicenter, prospective studies, two phase 3 prospective trials are ongoing.
