Use of Esmolol in Neuroanesthesia and Neuro Intensive Care

Çukurova Üniversitesi, Tıp Fakültesi, Anesteziyoloji Anabilim Dalı, Adana, Türkiye Esmolol is a unique cardioselective β(1)-receptor blocking agent with a rapid onset and short duration of action (only 10 to 20 minutes). This effect makes esmolol very attractive for the treatment of hemodynamically unstable patients. Esmolol is also reported to exert antinociceptive and anesthetic sparing effects. Esmolol attenuates cardiovascular responses to intubation or extubation and control of the hypertension. It may also be administered as an intravenous infusion or in bolus doses. Esmolol blunts the cerebral blood flow velocity increase during emergence from anesthesia in neurosurgical patients.In addition, beta-adrenoreceptor antagonists provide neuroprotective effects after focal cerebral ischemia in experimental settings. In this review we assessed the safety and efficacy of the β1-specific, adrenergic receptor antagonist esmolol in hypertensive emergency, neuroanesthesia and neurocitical care.
