Osteoblastoma-like osteosarcoma is a rare variant of osteosarcoma accounting for approximately 1% of cases overall. Although, its histopathological findings are similar to osteoblastoma, different treatment protocols are used due to local recurrence and systemic spread potential for osteoblastoma-like osteosarcoma. Thus, differential diagnosis of osteoblastoma-like osteosarcoma from osteoblastoma has vital importance. Recurrence rate was 90% for patient who were treated with surgery alone. After the addition of chemotherapy to treatment protocol, 5-years survival rate reached to 30%. Current treatment protocol in patients with osteoblastoma-like ostesarcoma is radical surgery and chemotherapy. In the present case report, the management of a patient with osteoblastoma-like osteosarcoma on dorsal 6th vertebra was presented. No recurrence or metastasis was detected on 2 years follow-up.
