Unilateral Lumbar Pars Fracture in Amateur Football Players: Conservative Treatment Outcomes

DOI : 10.5137/1019-5157.TND.3537 AIM: While previous studies have generally focused on professional athletes or the evaluation of bilateral pars defects, this study aims to assess the conservative treatment outcomes of amateur soccer players with unilateral complete pars defects from both clinical and radiological perspectives.

MATERIAL and METHODS: In this retrospective cohort study conducted between November 2019 and July 2024, male patients who played amateur soccer and had no neurological deficits were evaluated. The conservative treatment protocol included temporary cessation of football activities, initiation of a regular physical therapy and rehabilitation program, and the use of a lumbar brace. During the treatment process, if bone marrow edema was detected around the pedicle in follow-up imaging, conservative treatment was continued; if a normal signal was observed around the pedicle, the treatment was terminated.

RESULTS: A total of 21 male patients were included in the study. The mean age of this patient group was determined to be 24.48 (16-34). A total of 22 unilateral pars interarticularis fractures were identified, including one patient with fractures at two levels. Following treatment, no bone fusion was observed in any patient; however, 19 patients (90%) returned to amateur sports activities.

CONCLUSION: Conservative treatment of unilateral lumbar pars fractures allows for the improvement of clinical symptoms and enables patients to return to sports activities. However, the lack of bone fusion highlights a significant limitation of conservative treatment. These findings suggest that conservative treatment can be an effective option, but alternative therapeutic strategies should also be considered to achieve radiological healing.
