Mevlana Üniversitesi, Tıp Fakültesi, Beyin ve Sinir Cerrahi Anabilim Dalı, Konya, Türkiye Traumatic lumbar spondylolysis is a stress fracture of the pars interarticularis. Usually, this lesion can not be detected in the radiograms. Therefore, the wrong treatment is often applied. Traumatic lumbar spondylolysis in young athletes is more common than in the normal population. These fractures usually heal with a brace, medical treatment and physiotherapy. However, patients who do not benefit from conservative treatment should be operated. In our article, a 28-year-old male cyclist who had a pars interarticularis fracture after falling is reported. Firstly, a brace and physiotherapy were used to strengthen the back muscles. After 12 weeks, the patient came again to our clinic. There were no changes in the patient’s back pain and the patient underwent surgery. This patient’s conservative and surgical treatment are discussed with the literature.
