Spinal epidural abscess (SEA), requiring emergent surgery, is a rare condition. A SEA is composed of inflammation and granulation tissue between the dura mater and inner wall of the spinal column. Although rare, a SEA usually occur swith advancing age and its incidence ranges from 0.025 to 0.03%. SEAs associated with diabetic ketoacidosis in older patients with uncontrolled diabetes have been reported.

We reported, in this paper, a rare case that underwent emergency surgery for SEA. A 69-year-old male presented to the emergency department with leg weakness for 2 months that had intensified in the last 15 days and we learned that the patient was hospitalized one month ago for diabetic ketoacidosis.
