Microsurgical Anatomy of Vertebrobasilar System Aneurysms

The vertebrobasilar system is an anatomically complex vascular structure. A detailed knowledge of the anatomy is required for a surgical approach to this region. Besides, a thorough evaluation of the preoperative vascular imaging studies is necessary to avoid complications during surgery. This region is a common location for dissecting and fusiform aneurysms. The arterial structures branch off multiple perforators to the vital neural structures and there are lower cranial nerves in close relationship with them. In order to prevent complications such as coma or severe neurological deficits, all perforators must be preserved. Therefore, in order to perform aneurysm surgery in this region that deserves surgical mastery, the first step is obtaining detailed anatomical knowledge.
