Surgical Approaches to Brainstem Lesions

Surgery for brainstem lesions is highly challenging due to the presence of critical nerve pathways and nuclei in this region, and the surgical management of intrinsic brainstem lesions has always been controversial. The aim of surgery in this region should be to perform any resection without damaging normal tissues and injuring the critical nuclei and pathways as much as possible. The anatomical relationship between the internal structures of the brainstem and the safe entry zones should therefore be well known. It is believed that the risk of postoperative neurological deficit is minimized if the right approach is chosen and surgical manipulation of these sites is performed by experienced surgeons. With the advances in DTI and DSI tractography, neuroanatomy and neurophysiological studies in recent years, surgery of the brainstem, previously seen as an untouchable region, has become possible.
