Atlantoaxial dislocation and rotatory instability, often caused by trauma, tumors or congenital malformations, is a challenging disorder of the craniocervical junction. Atlantoaxial dislocations and rotatory instability can be subdivided with many classifications. However, it is important whether the lesion is surgically reductable or non-reductable. Due to its anatomically deep location and complex structure, lesions of the atlantoaxial joint region are difficult to diagnose and treat. Effective strategies in diagnosis and treatment have been developed with the latest developments in medicine. Current operation techniques and new fixation instruments are widely used in atlantoaxial dislocation management. However, these high-risk surgical procedures should be performed by extremely experienced surgeons. In this review, we aimed to reduce the development of complications and increase the level of diagnosis and treatment by explaining the definition, classification and treatment methods of atlantoaxial dislocation.
