We present a case with nonneoplastic hydrocephalus who had bilateral fourth nerve paresis. Magnetic resonance imaging of the brain revealed aqueductal stenosis. Bilateral fourth nerve paresis may be a localizing sing in involvement of the superior medullary velum (the anatomic site of trochlear nerve decussation) by downard pressure from an enlarged third ventricle.
