Endovascular Treatment of Tentorial Dural Arteriovenous Fistulas

1İstanbul Üniversitesi-Cerrahpaşa, Cerrahpaşa Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalı, İstanbul, Türkiye Tentorial dural arteriovenous fistulas (TDAVF) are an uncommon subgroup of dural arteriovenous fistulas (dAVF). The clinical course of TDAVFs is more aggressive compared to other types of dAVFs. Most cases present after a hemorrhagic stroke. Only a limited number of cases are diagnosed prior to hemorrhage; therefore these cases are referred to early treatment due to the aggressive behaviour of these pathologies. Treatment has classically been based on microsurgery but the treatment modality has shifted to endovascular methods in the last two decades. This paradigm shift is mainly due to the widespread use of Onyx, a liquidbased non-adhesive endovascular material. This article describes the diagnosis, clinical course, several classification systems, and endovascular treatment methods of TDAVFs.
