Pathologic Markers for High-Risk Meningioma

Histopathologic and molecular markers associated with aggressive behavior in meningiomas have been updated in the 5th edition of the World Health Organization (WHO) Central Nervous System Tumors Classification. Meningiomas consist of 15 histological subtypes. As in other central nervous system tumors, the WHO grade is the most useful histopathological predictor of recurrence. Among subtypes of meningiomas, clear cell and chordoid are accepted as at least grade 2, meningioma. Some other histopathologic and molecular criteria are essential for other subtypes. Presence of increased mitosis and/or brain invasion is equivalent to grade 2 meningioma. Markedly increased mitosis or overt anaplasia or the presence of the TERT mutation or CDKN2A/B deletion are required for grade 3 meningioma. Accurate evaluation of these features depends on the optimal methods used, from the macroscopic sampling to microscopic and molecular investigations.
