Brain tumors are the most common solid tumors in children. The management of brain tumors in children has improved significantly, reflecting improvements in the classification of tumors, identification of risk groups within histological subsets of tumors, and the incorporation of molecular techniques to further identify tumor subsets. Although significant improvements have been made in the outcome of some tumors, the prognosis for other types of childhood brain tumors is poor. Here, we will provide an overview of the epidemiology of childhood brain tumors and the genetic factors associated with them, discuss the general principles in the diagnostic evaluation and management of these lesions, and review how management approaches have evolved for several of the most common subtypes of childhood brain tumors. We will discuss how the latest insights into the genomic changes underlying these tumors could lead to better treatments. In addition to reviewing the literature and providing an overview of the complexities in diagnosing childhood brain tumors, we will discuss advances in the treatment and classification of the various tumor types where progress is most evident and where improvements are lacking.
