Instability of atlantoaxial pathologies cause sudden death because of a compression over the spinal cord. Therefore atlantoaxial arthrodesis is life saving procedure. We operated a case of plasmacytoma of C2, probabl atravmatic and another one a can genital case, lead to atlantoaxial instability treated by using occipito cervical fusion and instrumentation. In this surgical procedure, Cl and C2 vertebra fixed to each other by the posterior interfacetal screw technique and there also attached to the occipital bone. This surgical technique provides more rigid fixation than the other methods. In this paper, this techniques as well as others are discussed, by reviewing the literature.
