Ondokuz Mayıs Üniversitesi, Tıp Fakültesi, Nöroşirürji Anabilim Dalı, Samsun 12 year old boy is presented who suffered from conventional manifestation of shunt failure, such as severe headache, nausea and vomiting. Computerized tomography (CT) and magnetic resonance imaging (MRI) of the brain did not show ventriculomegaly and periventricular lucency. Direct rontgenography revealed discontinuation of the distal shunt catheter on the chest. This patient affirm that increase intracranial pressure may occur in shunt dependency without ventriculomegaly and periventricular lucency on imaging of the brain, and conventional clinical symptoms and/ or signs may be the major clues of diagnosis. We presented clinical and radiological feature of the case and discuss under the light of literature. Anahtar Kelimeler : CT, MRl, shunt failure, ventriculomegaly
