Bakırköy Ruh Sağlığı ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi, II. Nöroşirürji Kliniği Cerebral localization ratio for Hydatic Cysts among whole Ecinococcus granulosis diseases is approximately %1-2. The treatment is surgical excision of cyst by Dowling Tecnique. Postoperative clinical status is good enough in cases which the cyst has been excised without any rupture. In this paper, even though total excision of a serebral hydatic cyst was succesfully performed without any rupture, unexpected worsening neurological condition of a patient is reported. Mass effect of the porencephalic cavity, developed due to total excision of the cyst, has been thought to be responsible for postoperatively progressing seconder optic atrophy. In case of any improvement has not been observed clinically, mass effect porencephalic cavity should be demonsrated by Magnetic Resonance Imaging technique and the treatment shuold be performed by shunting procedures between the porencephalic cavity and the intraperitoneal or the intraventricular regions.
