A 4-year-old girl with right frontoparietal traumatic acute subdural hematoma (ASH) in whom cranial computed tomographic scan demonstrated spontaneous resolution of the hematoma with clinical improvement in 12 hours is presented. Our case is the first case in the literature with early (in 12 hours) spontaneous resolution of ASH in childhood. With literature review and discussion of other cases in the literature, conservative treatment is advocated on cases with good clinical status and with thickness of the hematoma on radiological studies available to spontaneous resolution.
