Arachnoidal cysts represent less than 1 % of intracranial space occupying lesions. 25 to 80% of them occur in middle cranial fossa and rarely found to be bilateral. Arachnoidal cysts may be complicated with subdural hygroma and hematoma after a head trauma. Surgical approaches for arachnoidal cysts are controversial. "Craniotomy and fenestration", "craniotomy, fenestration and membranectomy" or "cystoperitoneal shunt application" are the surgical treatment choices.

By this report, we present a bilateral arachnoidal cyst of middle cranial fossa complicated with bilateral fronttemporoparietal subdural hematoma. The clinical, radiological and pathological features are presented with the review of literature. Anahtar Kelimeler : Arachnoidal cyts, subdural hematoma
