Ventriculo-peritoneal shunt placement is the widely-accepted choice of treatment for hydrocephaly. Other alternative surgeries include ventriculo-atrial shunt, ventriculo-pleural shunt and endoscopic third ventriculostomy. Because of low complication rates, the ventriculo-peritoneal shunt systems are usually preferred. The ventriculo-peritoneal shunt absorption problem appearing afterwords led to surgeon preference for different sites of the body in order to place the shunt distal catheter. When there is a problem with the ventriculo-peritoneal shunt placement, the ventriculo-atrial shunt systems may be used. We report a very rare case of ventriculo-atrial shunt malposition. Shunt distal catheters may alternatively be placed in the left brachiocephalic vein when the shunt distal catheter cannot be placed.
