Less than 1% of hemangiomas, the benign tumors of vascular origin, occur within the skeletal muscles and intramuscular haemangiomas localized to temporalis muscle are exclusively rare. In this manuscript the diagnosis and treatment process of a 21 years old female patient who were previously given long term medical treatment for headache and later diagnosed as temporalis muscle hemangioma is presented.Although rarely encountered, hemangiomas localized to the temporalis muscle may play roles in the etiology of chronic headache. The application of advanced diagnostic methods after the detection of local swelling in the head and neck region in the history and physical examination can prevent the possible delays in the diagnosis and treatment process. For definite diagnosis and complete resolution of symptoms, surgery is the primary method of treatment that should be considered. Due to increased risk of local recurrance capillocavernous type hemangiomas should be closely followed up in the postoperative period.
