Kocaeli Üniversitesi Tıp Fakültesi Nöroşirürji Anabilim Dalı, Kocaeli An intra-suprasellar arachnoid cyst was identified on the cranial MRI in a patient who presented with headache and progressive vision loss symptoms. Ventriculocystostomy was planned but the procedure was not successful because of small ventricles and the choroid plexus reaching the anterior chamber of the 3rd ventricle. Supraorbital keyhole craniotomy was performed and the cyst was visualized in the prechiasmatic region via endoscope. Cystocisternostomy was carried out via endoscope by cyst wall excision. Suprasellar arachnoid cysts are benign intracranial lesions and the treatment should be minimally invasive. We present our use of the endoscopic cystocisternostomy procedure via minicraniotomy in a case where ventriculocystostomy was unsuccessful.
