Surgical Treatments for Dystonia

Dystonia is a general name given to movement disorders characterized by sustained, involuntary muscular contractions. They can be classified according to the age of onset, aff ected body part or underlying cause. DYT gene presence suggests genetic inheritance in primary dystonias. Secondary dystonias are generally acquired related to various causes. Peripheral surgical methods have frequently been used particularly in the treatment of cervical dystonias. Although lesion surgery with radiofrequency such as pallidotomy and thalamotomy in the treatment of refractory primary generalized dystonia has been successfully performed so far, these have become less popular with the advent of DBS procedures. Some publications report that GPi DBS is also eff ective in primary segmentary dystonia, blepharospasm, cervical dystonia, Meige’s syndrome, tardive dystonia and some forms of secondary dystonia in addition to primary generalized dystonia. We believe that new target points and stimulation parameters will be tried in the near future in the treatment of dystonia and that combined treatment options will increase.
