METHODS: 222 patients diagnosed as cervical disc disease, in Bakırköy Ruh ve Sinir Hastalıkları Hastanesi 3. Nöroşirürji Kliniği between the years of 1993-2003, was assessed retrospectively. The study comprised 202 patients with sufficient documentation. Simple discectomy in 108 patients, discectomy with fusion in 59 patients and fusion with plate in 35 patients was performed. Clinical evaluation was analyzed due to Odom's criteria and radiological was done with dynamic cervical radiographs.
RESULTS: The clinical results of patients treated with simple discectomy were 61 % excellent, 24% good, 15% moderate; discectomy with fusion were %74 excellent, 11.5% good, 12.5% moderate; fusion with anterior plate 77% excellent, 16% good and 7% moderate. 75% segmental kyphosis was detected in patients treated with simple discectomy. Even though graft collapse occurrence was seen 14% of patients treated only with fusion, there was no significant segmental kyphosis. There were no graft collapse and segmental kyphotic angulation in patients who were treated with anterior fusion and plate.
CONCLUSION: The clinical and radiological results of discectomy with fusion, in the treatment of cervical disc disease, were found better when compared with simple discectomy. Simple discectomy is prefered especially for the acute soft disc herniations and surgery with fusion is prefered for the degenerative disc herniations.
