PATHOLOGIES THAT CAUSE “TETHERED CORD SYNDROME” AND THEIR SURGICAL REPAIR

Thirty-five children with tethered cord syndrome were operated on at the Department of Neurosurgery of Cerrahpaşa Medical School between 1990 and 1997. Intradural lipoma was the most common pathology which caused tethered cord syndrome. Twenty patients were under 3 years of age. Cutaneous stigmata of occult spinal dysraphism, weakness of foot and/or legs were the most common presenting complaints. Eight children over 3 years of age had urinary incontinence. The surgical outcome was gratifying in the majority of patients. Two children were reoperated in the follow-up period because of progressive neurologic deficits due to retethering. Tethered cord syndrome can result in gross impairment of neurological function, early diagnosis and adequate release of the tethered conus are the keys to successful management.
