Antero-inferior Cerebellar Artery Aneurysms and Surgical Treatment

The antero-inferior cerebellar artery originates from the lower third of the basilar artery at the level of the pontocerebellar angle. Anteroinferior
cerebellar artery aneurysms are usually located anterior to the brain stem at the level of the artery arising from the basilar artery
or cerebellopontine fissure. The arising point from the basilar artery and the course of the antero-inferior cerebellar artery may be variable.
Antero-inferior cerebellar artery aneurysms are very rare and make up 0.1-0.5% of all intracranial aneurysms. Clinical findings vary in patients
with ruptured and unruptured aneurysms. Patients with an unruptured aneurysm are usually asymptomatic, but in case of a giant aneurysm,
symptoms and findings can develop due to the mass eff ect. According to the localization of the aneurysm, surgery can be performed using
the retrosigmoid, subtemporal, transpetrosal, suboccipital, far lateral or combined approach. Endovascular therapy may be an alternative in
selective cases due to high recurrence rate.
