Recurrence in Chiari Type 1 Malformation and Its Treatment

Chiari type 1 malformation and accompanying syringomyelia is not a homogeneous disease group in which all cases have the same cause and pathophysiological process. One cannot therefore expect a standard treatment method to provide good results in each patient. The most significant problem that is encountered in the treatment of these patients is that the clinical symptoms and findings may not improve or may recur, and the syringomyelia may not shrink. Patients not benefiting from surgical treatment or who have recurrence of the disease might be suffering from various problems such as insufficient decompression, development of hydrocephalus, cerebellar ptosis due to excessive decompression, and craniovertebral instability. Furthermore, the first diagnosis might be incorrect and the patient might not even have the Chiari malformation.
