Our case contains significant similarities with intraosseous sacral schwannoma as a rare tumor itself. These kinds of tumors give late symptoms because of their slow growth. A patient who had been suffering from low back pain for the last three months was found to have a 4x3 cm mass with lumbosacral magnetic resonance imaging (MRI). The mass in the sacrum was a schwannoma. This mass did not show paraspinal extension or wide neural invasion, and was removed totally with the posterior surgical approach and the pathology diagnosis was schwannoma. This patient was discharged without any problems or neurological deficits. In such cases, MRI is an early diagnostic test and total removal and minimal harm are the primary aims.
