The Molecular Biology of the Blood-Brain Barrier in Brain Metastases

Several factors and mechanisms are involved in the development of brain metastases. A vast majority of brain metastases occur in lung and breast cancers and in malignant melanomas. For the metastatic cells to reach the brain parenchyma, they need to migrate through the capillary endothelial layer that constitutes blood-brain barrier (BBB). At the same time, BBB acts as a barrier to chemotherapeutic agents. The mechanisms underlying the interaction between the brain endothelial cells and cancer cells have not been clearly defined. Although the BBB acts as a protective barrier against cancer cells, at times it also plays an active role in their migration. In this manuscript, we review the mechanisms and signaling pathways that are involved in the transendothelial migration of metastatic cells.
