2Fırat Üniversitesi, Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalı, Elazığ, Türkiye AIM: To assess the effects of medical or surgical treatments on mortality in patients suffering high-energy head trauma (HEHT).
MATERIAL and METHODS: Patients who presented to the emergency department following HEHT between 2012 and 2016 were evaluated. Age, gender, Glasgow coma scores (GCS) at admission and treatment modalities of the patients were recorded. Based on the GCS, patients who were treated as medically or surgically were divided into two groups; GCS 8-15 and GCS ≤ 7. Mortality rates of patients treated medically were compared with those treated surgically in a period of three months.
RESULTS: A total of 1181 patients were evaluated in the study (942 male, 239 female, mean age: 26 ± 9.8 years). Medical treatment was used in 780 (66%) patients and surgery performed in 401 (34%) patients. GCS was between 8 and 15 in 721 (92.6%) patients and ≤7 in 59 (7.4%) patients who were medically treated. These values were 330 (96.4%) and 12 patients (3.6%) in the surgically treated group, respectively. The overall mortality rate was 8.55%, and the mortality rate of medically and surgically treated patients was 6.29% and 2.26%, respectively.
CONCLUSION: The mortality rate was higher in the medically treated group than those receiving surgical treatment.
