Unilateral Lambdoid Synostosis (Posterior Plagiocephaly) and Deformational Occipital Plagiocephaly

Plagiocephaly is the nonsymmetrical presentation of the calvarium, and generally occurs on account of a deformational or synostotic cause. If unilateral lambdoid suture synostosis is caused by a synostotic factor, it is called posterior plagiocephaly. Lambdoid synostosis forms 2-3% of craniosynostosis cases in general. It is often unilateral but the syndromic cases may present bilaterally. However, when the craniofacial abnormality is caused by a deformational factor, this deformational occipital plagiocephaly is also called positional plagiocephaly, which most commonly presents as a result of pressure on the parieto-occipital area of babies caused by lying in the supine position. The treatments and surgical managements for these two clinically and radiologically different craniofacial abnormalities are different as well. As in all craniosynostosis cases, the purpose of the treatment is to reshape the cranium and increase the volume. In this review, both types of plagiocephaly have been evaluated.
