Unruptured intracranial aneurysms are increasingly detected with the frequent use of imaging techniques in clinical practice. However, the large majority of unruptured aneurysms will never rupture and only 1 in 200 to 400 is estimated to rupture. Physicians are now often faced with the dilemma of whether to treat patients who present with an incidental finding of an unruptured aneurysm or to manage them conservatively. Once an unruptured intracranial aneurysm is detected, comparison of the short-term and long-term risks of aneurysmal rupture with the risk associated with the intervention itself should be carefully evaluated. Several factors need to be considered, including aneurysm size and location, the patient"s family history and medical history, and the availability of an interventional option that has an acceptable risk.
