Non-Aneurysmal Subarachnoid Hemorrhage

Rupture and bleeding of intracranial aneurysm is the second most common cause of subarachnoid hemorrhage (SAH) after trauma. In patients with negative four-vessel angiography for aneurysm or arteriovenous malformation, a repeat angiography is required to completely rule out such pathologies. Perimesencephalic subarachnoid hemorrhage is diagnosed in a subset of patients. No definite etiologic factor is found to explain the SAH in spite of thorough screening for a variety of rare causes in about 15% of the patients. This cohort of patients poses a challenge in the diagnosis and further management.
