Angiogram Negative Subarachnoid Haemorrhage: The Role of Repeat Angiography

Aneurysmal subarachnoid hemorrhage (SAH) has a high rate of mortality and morbidity. A number of patients with SAH on cranial
computed tomography (CT) have been found to have no pathology on vascular imaging studies. In some of these patients, the
hemorrhage was located in the perimesencephalic region and had a benign course. In other patients, the SAH was diffuse and could
have a malignant course. Vascular pathologies may not be detected in patients with a diffuse SAH on the first angiography due to
vasospasm or thrombus and small lesions may cause rebleeding. In order to prevent such conditions, angiographic imaging should
be repeated. At the same time, it should be kept in mind that spinal vascular pathologies and pathologies that are not detected on
angiography such as cavernomas may lead to subarachnoid hemorrhage. Spinal and cranial magnetic resonance imaging should
be requested from patients to detect spinal and cranial vascular pathologies.
