Endovascular Treatment of Infratentorial Arteriovenous Fistulae

The most common locations of arteriovenous fistulae are the transverse and sigmoid venous sinuses. They present with a variety of clinical features, ranging from benign bruits to intracranial hemorrhage and neurologic deficits. The presentation and natural history of these fistulae are largely determined by the pattern of venous drainage. Knowledge of natural history and careful study of the angioarchitecture by angiography is therefore mandatory for correct management of these lesions. With better understanding of the pathophysiology of intracranial arteriovenous fistulae and improvement in catheter and embolic material technology, endovascular treatment has become the treatment of choice for most of these lesions. The goal of endovascular treatment of intracranial arteriovenous fistulae depends on the clinical presentation, location, and angioarchitecture of the fistula. Indications with a focus on endovascular therapy and the therapeutic goals, approaches, and techniques for endovascular treatment are reviewed in this article.
