Surgical Treatment of Glossopharyngeal and Geniculate Neuralgia

1Kent Hastanesi, Beyin ve Sinir Cerrahisi Bölümü, İzmir, Türkiye Glossopharyngeal and geniculate neuralgia are rare in facial pain syndromes. The diagnosis of these facial syndromes is made clinically. As in other neuralgia, medical treatment is the primary treatment modality. Surgical approaches can be used in patients who are resistant to medical therapy and who have developed drug intolerance. Microvascular decompression and gamma knife radiosurgery methods are the current approaches for glossopharyngeal neuralgia. The arteries most frequently involved in the vascular compression in glossopharyngeal neuralgia are the posterior inferior cerebellar artery and vertebral artery. The arteries most frequently involved in the vascular compression in geniculate neuralgia are the anterior inferior cerebellar artery and the posterior inferior cerebellar artery. Section of the nervus intermedius for geniculate neuralgia, and decompression of the 5th or 9th cranial nerve based on the accompanying neuralgic syndrome are recommended for geniculate neuralgia.
