¹Medipol Mega Üniversite Hastanesi, Nöroşirürji Anabilim Dalı, İstanbul The relationship between motor cortex stimulation and pain alleviation has been known for years. Although the underlying mechanism of action has not been clearly established, favorable results have been reported for various intractable pain syndromes. Better results have been achieved particularly for deafferentation pain radiating to the upper limb and the various facial pain syndromes. The surgical technique consists of electrode implantation to the somatotopic motor cortex consistent with the anatomic radiation of the pain either epidurally or subdurally, and connecting the electrode to an internal pulse generator. Although the surgery has a risk of epilepsy, bleeding, infection, and hardware problems, such complications rarely occur. We discuss the mechanism of action, implantation technique and clinical outcomes of motor cortex stimulation surgery for pain in this paper.
