Pre-Hospital and Emergency Care in Traumatic Brain Injury Cases

Traumatic brain injury (TBI) indicates an injury to the brain itself and can occur without any signs of external trauma. Pre-hospital
management of patients with traumatic brain injury is critical and directly linked to clinical outcomes. In the period following local
cerebral ischemia, the disruption of cerebrovascular autoregulation, and an injury associated with cerebral edema, the brain is at
risk of secondary injury and focuses on limiting secondary brain damage while optimizing cerebral physiology. Secondary injury
greatly affects the clinical outcome, and this damage can be minimized if appropriate resuscitative efforts are provided. Pre-hospital
care providers are responsible for identifying both head and brain injuries, ensuring stabilization, and transporting these patients
to an appropriate care center. In the emergency department, the necessary care includes making the definite diagnosis of the
traumatic brain injury with or without extracranial lesions (especially with CT among the imaging methods) and triage to determine
the appropriate structure for the following care. In addition to the prediction of TBI severity, investigation of other relevant lesions is
of great importance in the emergency room.
