¹Çukurova Üniversitesi Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalı, Adana, Türkiye Cranial gunshot injuries represent one of the pathologies with the highest mortality rates in neurosurgery practice. Penetrating cranial gunshot injuries are often fatal with a death rate of nearly 70% at the scene of the accident. In the literature, there are research series with up to 90% mortality even when all treatment modalities are applied. Computed brain tomography is essential in the neuroradiological evaluation of cranial gunshot injuries. Angiography should also be performed in cases of suspected vascular injury. Aggressive resuscitation and surgery without wasting any time are vital when necessary. Prophylactic antibiotics and antiepileptic treatments should also be initiated in these patients. A low initial Glasgow Coma Scale score, the presence of abnormal pupil findings, advanced age, perforating injuries, and a trajectory crossing the midline are correlated with worse outcome in cranial gunshot injuries.
