It usually occurs after trauma with cervical hyperextension, especially as a result of spinal cord injury in the cervical spinal canal, which has narrowed in elderly patients. The central cord syndrome is manifested by severe motor deficits in the upper extremities compared to the lower extremities, sensory loss at varying levels below the level of injury, and bladder dysfunction. In the old years, when technological possibilities and surgical techniques were limited, conservative treatment methods were prioritized. In recent years, the results of surgery have become better thanks to technological developments such as high speed drills, ultrasonic bone cutter and microscope and increased surgical experience. Today, conservative treatment is recommended for patients with instability, spinal cord compression, and mild neurological deficit (ASIA-D). Surgical treatment is recommended for patients with instability and spinal compression. Surgical treatment is generally useful in the first 24 hours.
