The Etiopathogenesis and Classification of Dural Arteriovenous Fistulas

Their detection rate in adults is 0.16 per 100,000 per year. There is no consensus on the etiology. The etiopathogenesis includes sinus thrombosis, head trauma, intracranial infections, tumors, previous craniotomy, and hypercoagulopathies. In some patients, a cavernoma, cerebral arteriovenous malformations, maxillofacial arteriovenous malformations, hereditary hemorrhagic telangiectasia, or intradural or extradural arterial aneurysms may accompany the dural arteriovenous fistula. Dural arteriovenous fistulas in children are usually congenital. Venous drainage structuring in dural arteriovenous fistulas forms the basis of the Borden and Cognard classification system. There is also the DES (directness and exclusivity of leptomeningeal venous drainage and signs of venous strain) classification by Baltsavias et al.
