Dural Arteriovenous Fistulas of the Transverse-Sigmoid Sinus Junction
1Wisconsin Üniversitesi, Nöroloji Departmanı, Madison, ABD2İstanbul Üniversitesi Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalı, İstanbul, Türkiye A dural arteriovenous fistula (DAVF) is an arteriovenous shunt enlarging between the leaves of the dura or localized on the dural wall of the venous sinus, and is common in the transverse-sigmoid sinus junction region due to the anatomical features. Although its etiology is not fully elucidated, there are hypotheses on dural insufficiency in the literature. Pulsatile tinnitus and headache are the most common symptoms in the clinical course from asymptomatic cases to severe neurological damage. 10-15% of all intracranial vascular shunts are dural artery-venous shunts, and transverse-sigmoid sinus junction DAVFs constitute most of these pathologies. Venous reflux is the most important factor determining mortality and morbidity. Neurological dysfunction occurs due to intracranial hemorrhage secondary to venous reflux. Treatment modalities are conservative follow-up, arterial or venous embolization, microsurgery, radiosurgery, and combinations of these modalities.
