Surgical Treatment of Superior Sagittal Sinus Arteriovenous Fistulas

Dural arteriovenous fistulas constitute 10% of all intracranial arteriovenous malformations. They most often occur in the region of
the tranverse, sigmoid, and cavernous sinuses, but they can occur in any dural structure. Superior sagittal sinus dural arteriovenous
fistulas are very rare but aggressive vascular malformations. Treatment can be challenging because of the multiple arterial supply, the
presence of sinus thrombosis/occlusion, and the critical venous anatomy of superior sagittal sinus. Treatment is mainly determined
by the clinical presentation and fistula anatomy. Endovascular embolisation, open surgery, or combination therapy are the main
treatment protocols. The objectives of surgical intervention include excision of the fistula and pathological dural leaflet, and physical
interruption or obliteration of the arterialised leptomeningeal venous connection. In this review, we aimed to discuss the current
approaches in the surgical treatment of superior sagittal sinus dural arteriovenous fistulas.
