Radiosurgery for Cranial AVF: Indications and Results

Dural arteriovenous fistulas (DAVF) are composed of pathological connections between meningeal arteries and the dural venous
sinus or subarachnoid veins. The clinical presentation of DAVF results from hemorrhage or ischemia. Cortical venous reflux is the
critical prognostic parameter for clinical progress and treatment outcome. The outcome of patients with cortical venous reflux
is much more severe than those without reflux. Surgical or endovascular treatment of a fistula provides acute occlusion that is
much more appropriate for these patients. Radiosurgery obliterates the AVM by damaging endothelial cells and causes smooth
muscle proliferation and deposits of calcium and collagen. Radiosurgery may be a treatment option for selected cases with these
mechanisms.
