Three different targets are used for DBS and consist of the Subthalamic nucleus (STN), Globus Pallidus Interna (GPi), and the Ventral Intermediate (VIM) nucleus of the thalamus in PD. Prior to DBS programming, basic technical knowledge and experience in neurostimulation should be obtained. In addition, functional anatomy knowledge about the target nuclei and surrounding structures is required to understand the efficacy and side effects. As there are many options in programming, a systematic approach should be followed. Correct programming of the DBS and careful management of medical therapy and side effects are as important as choosing the appropriate candidate and target and correct electrode placement in the success of DBS. In this article, technical information about DBS programming, basic programming principles, approach to possible side effects, and postoperative management of the patient with the anatomical targets commonly used for DBS in PD, particularly in STN, are presented.
