Deep Brain Stimulation Organization and Ethical Obstacles
1Mersin Üniversitesi, Tıp Fakültesi, Nöroşirürji Anabilim Dalı, Mersin, Türkiye2Mersin Üniversitesi, Tıp Fakültesi, Nöroloji Anabilim Dalı, Mersin, Türkiye In the last 25 years, high frequency deep brain stimulation (DBS) has been developed to replace the classical lesioning methods previously used in stereotactic and functional neurosurgery. This method has proven its effectiveness on the basis of its reversibility and adaptability. The method was initially used in various target areas for Parkinson"s disease and essential tremor. It has now been used for symptomatic treatment of epilepsy, dystonias and cluster headaches, and more recently psychiatric disorders. Although DBS is a widely accepted treatment for motor disorders, it remains an invasive and expensive procedure. The ethical and social challenges of DBS need to be further examined in the light of the organization of experienced teams. Significant ethical and social challenges exist in the practice of DBS, particularly in patient selection, informed consent, resource allocation, and public understanding. In order to identify and characterize existing and emerging issues in the use of DBS, the neurosurgical literature on DBS, as well as interdisciplinary medical ethics, and the related psychological and sociological literature were reviewed. Reviews of ethical and social issues related to DBS highlight that many important, if not invincible, challenges require much closer attention.
