They make up more than 30% of all primary brain tumors. Maximal safe resection should be the treatment method for symptomatic and rapidly enlarging tumors. More than 80% of these tumors are grade I, and the remaining are more aggressive grade II/III tumors. All intracranial meningiomas have the risk of recurrence, including the more benign counterpart grade I tumors. As with primary meningiomas, recurrent meningiomas are treated with maximal resection followed by radiation therapy; again the functional status of the patient should be the primary concern. When surgical and radiotherapy options are exhausted, systemic treatment modalities are applied. However, the available standard cytotoxic agents and hormonal and molecular therapies do not improve the overall and progression free survival rates. Thus, further studies are warranted for the development of systemic treatments for the patients for whom the surgical and radiation therapy options can no longer be used.
